About CovenantCare Home Health and Hospice
Serving the needs of communities since our inception.
The CovenantCare Home Health and Hospice family includes Covenant Living Communities and Services located across the United States and CovenantCare Home Health and Hospice located in California and Illinois. Our family of health communities grew from the ministries of the Evangelical Covenant Church (ECC) and its 127-year Christian legacy of community service and health care outreach.
In 1886, the Swedish Evangelical Covenant Mission Friends, an outreach of the ECC, founded the Home of Mercy in Chicago. The Home of Mercy was a multi-purpose entity that served as a retirement home, hospice, refuge, and hospital for local immigrants during a time of increasing social needs in the community. From this legacy of care grew the mission of CovenantCare Home Health and Hospice.
Collaborating with hospitals, physicians, and residential care facilities to provide Medicare–certified health care, personal care, and hospice services, CovenantCare Home Health and Hospice focuses on continuity among all medical providers. This continuity allows us to provide the best quality and patient-centered services to our clients. Research reveals that individuals heal faster in their home environments. CovenantCare Home Health and Hospice responds to this research by delivering quality care in a cost-effective way, by being mobile to patient needs.
We come into communities with a legacy of compassion armed with a future of health care innovation.
Mission
Our mission is to promote and maintain the well-being of individuals in their homes. Committed to excellence, we maximize their physical, psychological and spiritual well-being while celebrating God’s gift of life.
A change in health status should not mean a change in the quality of your loved one’s life. CovenantCare Home Health and Hospice is an award-winning, Medicare-certified Home Health and Private Duty Home Care services provider. Prepare for a quicker recovery when your loved one stays within the familiar comforts of home.
Your loved one’s health is our house call. We provide health care answers that are tailored to needs and ability. Our team of experienced professionals provides care across a broad spectrum. And, according to physicians who provide care oversight in collaboration with our caregivers, we are their eyes on patients. A physician said, “I cannot imagine caring for seniors without the collaboration and services of home health care.”
